This is our intermediate level. It provides individualized coaching and transition planning to help youth create measurable progress toward adulthood.
Primary Goal:
Create stability and measurable progress.
Includes:

✓ IEP/504 coaching
✓ Career exploration
✓ Life skills planning
✓ Resource referrals

Annual Capacity:
30 youth annually

• Youth experiencing moderate transition barriers
